HIPAA Notice of Privacy Practices

Aesthetics MD is a HIPAA-verified practice. We protect your Protected Health Information (PHI) as required by the HIPAA Privacy Rule (45 CFR Parts 160 and 164).

• How we use your PHI: for your treatment, for payment activities, and for our health care operations. We share only the minimum necessary information, and only as the law permits or requires.
• Your rights: you may request a copy of your records, ask us to amend them, request an accounting of disclosures, ask us to restrict certain uses or sharing, request confidential communications (for example, contact only by a specific phone or email), and receive a paper copy of this notice.
• Authorizations: we will not use or share your PHI for marketing, or sell it, without your written authorization. You may revoke an authorization at any time in writing.
• Breach notification: you have the right to be notified if a breach occurs that compromises the privacy of your PHI.
• Complaints: you may complain to us directly and to the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
